Requirements for Legal claims sworn before a Commissioner of Oaths / Notary Public: Change of name, nationality, date of birth etc.
- Statement declared before a Commissioner of Oath giving reasons for the change, etc.
- Cover letter from the Judicial Service, Accra.
- Cover letter from the Ministry of Foreign Affairs.
- Certificate from requisite Ministry or organisation which approved the change
- Original and copy of Gazette in which change was published
- Endorsement from both Ministry of Foreign Affairs and Spanish Embassy in Accra
- Copy and original of Passport
- Personal appearance at Embassy
- Fee: €50
Requirements for Birth Certificate, Police Report, Marriage, Divorce Certificates and other Legal Certificates
- Cover letter from the relevant issuing office
- Cover letter from Ministry of Foreign Affairs, Accra and Spanish Embassy in Accra
- Original and Translated copies of documents.
- Endorsement by the Embassy of the Kingdom of Spain in Accra.
- Personal appearance at the Ghana Embassy, Madrid
- Copy of Passport
- Fee: €50
Requirements for Endorsements of Certificates for use in Ghana
- Endorsement by the relevant Ministry
- Endorsement by the Ministry of Foreign Affairs in Madrid
- Certified translation into English
- Fee: €50
All Applicants are to submit completed registration forms personally at the Embassy, with no exceptions.
